An enemy would have to specifically decide to target a Retainer instead of a PC. So, the Retainer being attacked would be the one who has caused the most trouble for the enemy.
I.e. The enemy hasn't been able to hit the PC even once, so they purposefully attack Mary, the Retainer who has been providing cover.
At that point, the enemy is basically just giving up one of their attacks to try to remove some bonuses from the PC, rather than attacking the PC directly. You would resolve that with a To-Hit roll and/or damage roll vs. the Retainer's AC and HP respectively.
If the Retainers are causing problems, but there's no reason to attack one of them in particular (perhaps because they're all performing the same task), just roll a d4 to decide which one of them is going to take the hit.
Or, just make up your own rule at the table. That's what I usually do.
